Six Common Errors in Evaluating for Relapse in ANCA Associated Vasculitis

This morning Dr. Peter Merkel presented a wonderful talk on ANCA Associated Vasculitis (AAV). He identified the following six errors when evaluating for relapse in AAV.

  1. Assuming symptoms are due to AAV

  2. Assuming rise in creatinine is due to AAV

  3. Treating the ANCA titre or ESR/CRP. On the whole, there is currently no clear evidence that treating an isolated increase in ANCA is beneficial to patient outcomes.

  4. Not considering new/changing co-morbidities. Patients develop other medical issues and co-morbidities over time. The new neuropathy may be due to their diabetes and not vasculitis.

  5. Not considering the effect of treatments. Don’t forget to consider the side effects that treatment may have. 

  6. The original diagnosis of AAV was wrong. Remember to reconsider the diagnosis when the clinical situation does not make sense.
